What happened to Clayton Family Practice?

    We are in need of our medical records and the phone number is saying it has been disconnected. Anyone know what happened to this Practice or how we can get our medical records?

    Yes, it was. I just spoke with her daughter and she stated that FastMed now has everyone's records. So glad to know this as others may need their records too.
